General Information

Insurance:
We accept most of the major insurances in the valley area including workman's compensation.

Please feel free to call us or check with your individual insurance provider for the services covered under your particular plan.

Payment Options:
Cash, check, credit card

Appointments:
Please arrange for a referral from your referring physician. Bring MRI, CT scan, X ray, EMG/NCV and relevant reports to the appointment.

Emergencies:
Please go to the nearest emergency room, or dial 911.

Additional Information:
In order to make your visit to our office efficient and comfortable, we ask that you complete some paperwork at home.

For Patients:
There are three different forms. You must complete each form (to the extent possible) prior to your visit. Call us at (702) 739-8323 if you have questions.

1. Questionnaire: This form has critical initial health information needed for us to take good care of you.

2. Pre-authorization request: This form is to ensure coverage of your visit by your insurance company. Complete the insurance section within.

3. HIPAA form: This form ensures compliance with patient privacy.
